To calculate your skill score you just have to add the relevant proficient score to the proficient score of your character. Suppose your relevant proficient score is +2 and your proficient score is +2 then your skill score becomes +4.

Passive perception score

To calculate your passive perception you have to add your passive perception skill with a+2. The score of passive perception skill is 10 at the initial level of playing. So, to calculate your passive perception score at the initial level you just have to add a+2 with the perception score.
